- Open the assessment questionnaire.
- Place a check in the box next to the questionnaire item(s) for the response option to be added.
- Select Apply Scale.
- Choose the applicable scale to be applied to the questionnaire item(s).

Scale applied successfully confirmation will display.
- Select Ok.
- Select Save.
